Where can I find an excel template for creating Outlook contacts

I want to add a bunch of contacts to Outlook. I thought I could create an
excel template with fields such as name, address, phone, birhtday, etc that I
could send to my contacts and ask them to fill in their information. Then, I
would import the Excel file into Outlook.

Should be easy enough to build from scratch.

Export your existing Contacts to a CSV file using the File>Export.

Map just the fields you are interested in. Doesn't matter if empty or not, what
    you want is the headings.

Open that file in Excel and all the headings you mapped during the export will
    be entered in row 1.

Save just the sheet with the headings as a workbook and ask others to fill it
    in.

Note: if you map every field from Contacts you will have headings to column CN
