Have a look at this example.....its PDF, but you can see at the
bottom, the table/cells are present allowing the end user to input the
data.

Looking for advice, someone needs me to make them a cue sheet ASAP!

Well, you could do this in Excel, but wouldn't Word be more
appropriate?
