    A Form and an Excel page that will play two ways...

    I have approached this one other time here but with a different thought process for part of it. Thus I decided that it might be easier to start a new thread so as to not confuse the two. I will note that this is far more advanced than anything I have done in the past and likely do not have the vocabulary for describing all the areas I'm trying to express... I will not be offended if you treat me like a total nube on this one as I feel that way already.

    So to the best of my ability here is the description. I have a set of clients that are contacted on a monthly basis. Those contact attempts and results have to be recorded on a very specific PDF form that can not be changed.

    Currently the excel sheet is basically just an on screen reference and the PDF form is completed by filling in the blanks by hand and then scanned one at a time.

    Here is the desire:
    1 The excel contact sheet would contain, a form page in a PDF format for each contact within the excel document for each individual contact.
    2 The form page would populate given contact information through "mail merge" from that row on the excel document into the given areas of the PDF form.
    3 The remaining areas of the PDF form would be a fillable PDF however it would be ideal for it to then record those inputs back into given parts of the excel sheet. (So not changing the mail merge items, but the blank sections would be recorded into the excel document.)
    4 The next step would be to have a way to print all the populated PDF forms (preferably not one at a time).


    Some variables in the mix of things or things that seem to add complications.
    A. Some of these contacts have numerous contact modes. Being able to choose the most reliable from the excel sheet to populate onto the PDF is much needed. (we can not change the PDF form so 3 individuals is max but on occasion some of them actually have many more possible) It would be ideal if without changing the excel sheet there would be a way to choose which ones populate onto the PDF form.
    B. As the PDF form is actually completed and contact has been made it needs to reflect that into the excel sheet in order to mark it as a completed contact.
    C. File size for storage is not a great concern but we are talking about the spreadsheet containing between usually about 100 contacts.
    D. Some of the users have basically zero excel ability and to ask them to move things around will likely cause disasters.
    E. Things that can change are the excel page can be expressed in a different way if needed though it would be nice to have the majority of contact info viewable on screen without scrolling left to right and with keeping sizing up to make reading it easy.
    F. All those things I have yet to encounter in this to make it work.

    Attached is two items,
    One is a model of the current excel sheet, it was put together this way for ease of seeing most of one contact within the parameters of the screen and occasionally printed page. (some categories have been removed just because they are not pertinent to this application).
    Second is a word doc I threw together to give a rough idea of the PDF page that is required.
    Those things marked with Yellow highlighter are things that would normally be checked or circled on the form.
    Things in Red are items that would hopefully be mail merged into the PDF (other categories would also need to be added to the mailmerge for other parts of the form not shown on either the form or the excel here so the excel nor the form are complete here.)
    Things in Blue are those things that need to be moved from the PDF form into the excel sheet.
    Things in Green are those things that just are recorded into the PDF form for print and to be saved as part of the PDF.
    Things in Bold Black are just notes to clarify.
