    Extract any changes in 5 columns in Array of Data to produce Table

    Large array of data, would like to auto look in 5 specified columns for any changes (in any of these columns) and then extract the data in that cell plus cells to the left and right into a table form.
    Attached is a sample I created to illustrate.

    The combination of entries in 5 columns means every row is unique. You would need to ignore the date column in your example.
