I am looking for a formula to return the following:
Number of people under 5 years old admitted into system as type A before 10/10/2010 (Inclusive).
The total should be 2.
Doe, Jane appears 3 times and Doe, Mary once. Therefore 2 people.
I need to ignore the duplicates but taking into consideration that more than one person will have the same surname. I must not add any extra column to the spreadsheet.

Welcome to the forum.
There are instructions at the top of the page explaining how to attach your sample workbook.
A good sample workbook has just 10-20 rows of representative that has been desensitised. It also has expected results mocked up, relevant cells highlighted and a few explanatory notes.
