Data input form - Help

Hi all, I'm totally new to using VBA and I'm just looking for help with some really basic code.

I want to create an Excel user form that will allow input to 7 fields of data as follows -

6 fields each with a separate and different set of options from pre-defined lists
1 field for user inputted text

I'd like this info to then feed to a spreadsheet which can be accessed later so that the data can be copied and pasted elsewhere.

Ideally I'd like the user to be able to open up the file, be greeted with a form to complete, click a button on the form to transfer the data entered and then move on to the next record. I'd like another button which will open up the raw data sheet so the entered info can be edited and then transferred via copy/paste. I've attached a picture of the basic layout to give an idea of what I'm looking at.

I've looked online but can't find any resources that fit the bill and I'm clueless when it comes to VBA.

I appreciate that It is a similar form and I have tried to copy what I already have from the other form and change it round to fit my requirements but I can’t seem to get it right, the minute I add additional drop-down boxes it all stops running

Please attach a sample workbook (not a picture or pasted copy). Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

Remember to desensitize the data.

Click on GO ADVANCED and then scroll down to Manage Attachments to open the upload window.
