Error on conditional formatting with gold stars

Hello,
I am using Office 2013 and I have an odd error.
Please see the images below. I am trying to add a gold star for each rank of one through five. I must be doing it wrong because sometimes it also adds a star to a rank of six. How do I correct this error? I've heard that sometimes this can be done with percentages instead of numbers but if sop, what percentages?
