Pull data from another table, only if certain criteria (name) is met

I found this spreadsheet in this forum and I'd like to add a column in column D of points tally that states when the next absence falls off for each person listed.

What I'd like to do is be able to pull this information and create a performance tracking sheet that would automatically populate based off the information in this spreadsheet, so if you could also explain what each step of the formula you use means that would be amazing.
