    Fill out data from one sheet to another sheet

    I am using two sheet one is Form and other sheet is DATA so in Data sheet I have Name,ID,Title so what I need is so in sheet Form B2 when I select name by drop list to fill out cell C2 and D2 from sheet DATA. Please see attachment .

    Just use either VLOOKUP or INDEX MATCH using the number in column A as the matching value.
