Is this an internal or external report? If internal you can deliver the following: I would group by Field / Event and show the percent of changes. The top 2 have 4, the rest have 1. I would also show the time between the changes. This can identify creation errors vs changes needed. The values flip from 1 to 2 back to 1 for Information Sharing Risk Rating at the onset. I would have a change count by year to show changes have been abnormally high for 2017. Can you tie back cost of changes to the changes (resources needed)? Where the changes came from?

For external reporting, the changes right after implementation may want to be avoided. I know this is sample data but there isn't much to work with. Who is the report for and how often will this be requested? Those are things to keep in mind.
