Find were user from is getting data

Hi

I have been trying to adapt some code to suit my needs.
The add record works as expected and puts the data into sheet 2
The view/ edit button works but the names inside are still what the old code had, i cant see where to change this form


any Help would be appreciated

Unprotect the sheet, then right-click each button to be able to edit the button text.

Its not the button text i want to change, when you click the button another find box appears
if you the type fed in it brings up the edit record form, its the name of the edit boxes that i want to change but i cant fine out where they are.


i can't find out where this user form is to change the ( plan number 1 etc)

hope that makes sense
