Getting unique values based on Multiple Criteria

On the attached spreadsheet I have a table of data (Sheet 1). I need a fast and efficient way to pull the unique values in the Report column based on the criteria listed on the Records Tab. I would want the unique values to be on the records tab starting in cell A8. So basically the criteria is for unique reports worked with the date range provided for the ID number listed.
