Look up INDEX issues

Hello,

I'm new to indexing and i'm having some issues.
I would really appreciate some guidance, as I've tried a couple of goes but I keep getting errors.

I have this document and would like to be able to enter a Student ID in B2 on the "check" sheet and for all the activities and dates related to that ID be pulled from the "entry" sheet (column A and B).
