Why not use proper data entry? That is each column represents a field and has a header. Each row is one record.
This format is the easiest to work with and can produce many different "report" type results rather than entering into a report type form which is difficult to gather statistics from or to produce other reports.
The format would be something like the following which would allow the creation of many different reports.
