Form for data entry

Hi there,
How do I (or is it possible) to have a button on my spreadsheet that will bring up a pop-up window...
On which, is a drop down list of all the names I have in a column A of a table on sheet 2, and then besides a name in that pop-up I can add some data that will be added to a later column (column X) in that table?
