Search for a column and fetch values for corresponding values from other column

Hi All,

I am totally novice to excel and this forum.Could someone help me in fetching data for corresponding columns using VLOOKUP /INDEX.I have attached my original sheet and required sheet.Any help would be grateful.

The "sample" is so large that I did not manually check any of the results. I changed the content of cells A1 and A48 to provide identical matching terms. is this what you had wanted?

I worked with yours and its working fine too.But I've added one more column and search is based on that.I am not able to get data for some records in result sheet as date can't be formatted properly.Could you pls help me on this.Please find the attached sheet with this post.
