Need help to make a data entry form.

i have some .tiff file.
and i need to input data from .tiff to excel.
Using a user form i think this job can done easy.
i can make some little form. but its contains more field. I am getting confused how can i make it correct.
I have attached my excel sample.
Can any one give me some suggestion.

I Can not upload .tif file. provide invalid file error.
