Help with formulas / Layout

Hello All.

I have been trying to find the best way to format some data that is coming to me in the format of column B of the attached file. I am trying to write a formula to work on a large dataset and make it look like columns H through N.

I tried doing a vlookup to do it, and maybe I had set it incorrectly, but it really did not work for me.... any thoughts are appreciated! The one caveat i have with the listing is that not all of the addresses have the same info listed. so a website may be missing, or fax number, etc... but still trying to make it link up correctly.
