Need help finding a formula to give me the weekly results in a range and start/end dates

Need help to find a formula that will give me the weekly results from a range and pull the starting and ending dates for each week and then list it in a new table.
The table on the right is just text i filled in for what I wanted it to look like as an example. I need the Week numbers and the start and end date of each week to populate the table.
