Pull data to summary page

Hello, I have a sheet full of data, and I want to lookup once piece of data based on four other inputs.

For example, If someone is from North Carolina, is 22 years old, is an analyst, I want the summary page to populate that the person's name is Craig.

I made an example Excel template to show exactly what I need. The user fills-in several known items and then the formula should search for an "answer" in the raw data tab to populate for the user in the summary tab.

Hey, sorry for taking so long to get back. I actually need it in a different format, and I can't seem to get it to work with your equation.

With the attached file, the user will enter in the first two prompts on the Generator tab, and then the next three related boxes will be filled-in, by looking up the first two columns on the second tab with all the data.Generator.xlsx

Can you possibly help with this?
