    Index match with a twist

    Hi,
    Im trying to do an index match to bring back a % although im having trouble achieving this.
    Attached I have two tables, one with yellow around it, this table is where I would like the formulas to be to bring back the data I need. The data I need is from the table with red around it.
    I have filled it in manually and colour coded it, with the % I am trying to get from the table.

    I need to look up the date the type and the ID

    Can an index get this data via a formula?

    Please see attached file with formula in B6.

    =INDEX($B$17:$Z$20,ROWS($B$17:B17)*2,SMALL(IF(($A$17:$A$20=$A6)*($B$17:$Z$17=B$3)*($B$16:$Z$16=B$4),COLUMN($B$16:$Z$16)-1),1))


    ...confirmed by pressing CTRL+SHIFT+ENTER to activate the array, not just ENTER. You will know the array is active when you see curly braces { } appear around your formula. If you do not CTRL+SHIFT+ENTER you will get an error or a clearly incorrect answer. Press F2 on that cell and try again.

    Here is another way....Enter in B6 and copy across and down
