Multiple IF THEN ELSE statements from two cells

EDIT, the title should say multiple cells, not two cells.

Hello,

I need help with a massive IF THEN ELSE situations, I've done a max of two IF THEN ELSE statements but nothing this massive and I'm beginning to think it just can't be done except manually.

I've attached a weight example spreadsheet. There are five columns, Starting Weight, Mid point weight, Midpoint weight part 2, ending weight and what I need to figure out are the Next Actions, which is the last column.

Here are the scenarios, which need to go in order from First to Last:

1. If starting weight is greater than or equal to 200 and end weight is greater than or equal to 200, this should be labeled as One on One
2. If ending weight is greater than or equal to 200, this should be labeled as Conference Call
3. If ending weight is greater than starting weight, this should be labeled as Examine
4. If ending weight is starting weight, this should be labeled as Home Visit
5. If Midpoint weight is greater than or equal to midpoint weight part 2, this should be labeled as Examine Mid Checkpoint
6. If there is a decrease in the Midpoint Weight and Midpoint Weight Part 2, this should be labeled as Examine Mid Trainer
