    Index Match for multiple criteria within a row

    I'm having an issue using INDEX MATCH and trying to look up a value in a column that has dependent criteria within a single row, as well. I've attached my spreadsheet for reference. I need a cell to return a value based on the input from column A (either 6 or 6.25) and column B (12, 14, 16, or 18) at a certain efficiency level (given in row 2). Any help would be appreciated.

    New quick method:
    Select Thread Tools-> Mark thread as Solved. To undo, select Thread Tools-> Mark thread as Unsolved.

    Or you can use this way:

    How to mark a thread Solved
    Go to the first post
    Click edit
    Click Go Advanced
    Just below the word Title you will see a dropdown with the word No prefix.
    Change to Solved
    Click Save
