    LOOKUP formula with variables

I need to know what formula i could use to look for a name, but when it finds the name it brings other info from cells within the same work sheet and sends the info that was recieved to multiple locations within a different worksheet. Here is the info i am hoping to find Names, #, Date and Task
I want it to look for the info on worksheet MONDAY the names will be in A8:A23, the number will be in cell M4, the date will be in cell B3, and the task will be in cells B8:J23(going along with the name that is in A8:A23). when it finds this all this data i would like it to put on anothe workshet named TIMECARD. On this sheet the names that it finds need to go in different locations and canot be duplicated. the list of cells for names are B4,N4,Z4,B50,N50,Z50,B96,N96,Z96,B142,N142,Z142. The #A6:A37, the dates go in C6:I6 and the task goes to B6:B37. it is hard for me to know what needs to be done because all the information needs to match depending on the names in cells MONDAY!A8:A23. Thanks in advance.

It is usually easier to convey all those cell references, and what is in them, by attaching a sample workbook - just remove any sensitive information first.
