To find Weekly High and Low for the company

Hi,
I have data in column A to Column E in which name of the company,High price,Low price,date and week is mentioned.
What my requirement is that if I type in Cell H1 name of the company then in H3 I should get high price of that company for that particular week and in H4 I should get Low Price of that company.
For better understanding I have attached excel sheet.
