Vlookup to create auto-fill

Apparently it's been a long time since I've done vLookup!

I'm trying to create auto-populate fields with text from a drop down menu. I've attached what I'm trying to do. Depending on the selection from the drop-down (red field), I want either text from yellow or blue cells to auto-fill in the green fields, blank if they choose the uncolored option.
