Formula to index each column and return whole number greater than zero below Yes OR No

I need a Formula to index each column and return whole number greater than zero below Yes OR No (please see example)

The problem was recently solved with thanks but as I have expanded the spreadsheet I need further help.

IF OR functions I can do and they will work but the sample spreadsheet has limited rows compared to the model I am developing.

I need a column index, return whole numbers only, at values greater than zero where they sit below the word Yes OR No.

Help once again appreciated from this forum.
