Indexing and Lookup

I have several years worth of data (Sheet7) separated in 3 sections (I had to remove 2 years since the file is too big) and I would like to extract the amounts based on the account number and the month/year found in Sheet5. How do I do this using a lookup or index formula? I am trying to compare two different months/years side by side.
