    Sliding Fee Schedule and Formulas

I've attached the sliding fee schedule that we're going to be implementing. What I'm trying to figure out is if there is a way to have a spreadsheet automatically figure out from the individuals income what percentage rate should be used by looking at not only income but also status. I've attached the fee schedule showing the range and categories. (ex. If an individual has a family+4 and monthly income is $3350, it tells me to use 3%). I've added from my initial posting thread (sorry, couldn't find my original) to not just figure out one category but to figure out which category to choose the percentage rate. I was thinking maybe the VLOOKUP but I'm not familiar with it. If it's not possible, please let me know.
