Need Help with Index and Match Formula

As you can see in the file. Cell A19 has the result. Cells D19 and E19 are the drop down lists from the ranges from A5:A13 and B5:B13. I tried using the index and match formulas to give me the result, that is the Unique Code.

Problem: Whenever I choose the Item and 1000 counts in D19 and E19 the result in A19 is an error. However whenever I choose the Item and 100 counts in D19 and E19 the result comes out fine.

Can anyone tell me what the problem is? What I am doing wrong? I am getting so frustrated!! Please HELP!!!

    Thanks in advance!!!

I have also attached the file for your convenience.
