Index with imbedded multi-criteria Match

Hi. I'm trying to use an match formula imbedded in an Index function to find a row which has 2 different parameters. I'm use the following formula but am getting a #N/A error return. Please can someone spot what I'm doing wrong?
I've also attached an extract (image format).
