Recording polling preference data

Hi Guys
I want to poll a couple of hundred people for their preference to attend a event and have a couple of dates available.
I have looked through the forum and found a sheet provided by mikerickson which I have modified to get the polling results that I require.
I was wondering if would be possible to add a third work sheets that actualy captures each individuals voting preferences. I am now totaly out of my depth and would be grateful for any help that you could offer with this.

What I really like about this sheet is once someone has voted their name is removed from the list of available voters so I would want to retain that functionality.
