Postcode Match: Cross Reference from another list

Hope to solve this problem (have checked the archive - have found similar examples & problems but the solutions offered in these did not seem to work)

Simple case of wanting to match a postcode County and Country from a table containing this data.

I think it is a combination of INDEX and MATCH, entered as an array - although, I have also tried an OFFSET - but as you can see from the snapshot this is not working.
