Formula for giving all the results for a specific name

i want to my formula for "Bon Pegawai" its from "Kasbon Sheet"

=INDEX(Kasbon!C2:F455;MATCH(Adhe!$C$1;Kasbon!D2:D455;FALSE);1)
but its just for name "Adhe"
not for another name

and if i use another name like Anti
im using that formula
that just get same answer its like
