additional variable in formula

Hey all,

Hopefully someone can help me. I have attempted a couple of variations of Index, Match array formulas to bring back an answer for 3 variables, but I am clearly not sucessful here. I have attached the worksheet.

In the example of cell E3, I am looking at the Product Line, in this case "Lost", and the Country, of which i have 5 possibilities. The final variable in this equation is the date.

If anyone knows a formula that can say, if you select x month, and y country for z Product you will get this number, it would make my whole month!
