Date and Dollar Concatenation populated in one cell.

I am seeking to concatenate a Date and Dollar Amount in one cell in the same format.

    Date Dollars
    1. 10/1/2013 1. $7,335,894
    2. 10/1/2014 2. $8,376,475
    3. 10/1/2015 3. $8,510,385

To be displayed in one cell in the

    1. 10/1/2013 $7,335,894
    2. 10/1/2014 $8,376,475
    3. 10/1/2015 $8,510,385

    I have tried to concatenate the two, (= D4 & " " & " " & G4) but I come up with this: 41548 7335893.57285633 (example one)

Is there a way to format the output of a formula that concatenates two values?

This is really rubbing my like a stucco bathtub and I need help!
