VLOOKUP not returning correct information from given array

On the attached workbook I am trying to have information displayed in B5 when a selection from D4 is chosen from the list. The array is located at A69:B92 and the information that I want to be displayed is 2nd (description of each exercise B69:B92). It is currently showing some information but it is either the incorrect information for that exercise or it displays #n/a. I have used the same code multiple times and in the exact order and it works on fine then but just not now, so I am lost. I apprecaite any assistance.
