Formulas And Tables Form for Tracking Item Prices

Hello,

I need a table form and formulas that can help me to track a price item.

for ex: a supplier offered me a price for an item = 20$
second day he offers me for the same item a price = 30$

I want to track the price such putting a cell where i type the name of the item will give me the name of the supplier and the price for each day.
