Calculate dollar amount paid if between a to and from amount

Hello, I have a chart that has in dollar amounts a "Wages in base period" Starting in row C and ending in row E and if your within that range then it would display the "Weekly benefit rate" OR if below $2,200.00 it would display the words "INELIGIBLE" both in (Cell I5). The number that would be entered to see what rate you would get would be in (I4) How would I do this?

Example:

If I entered $5,500.00 into I4
it would display $58.00 in I5

Example 2:

If I entered $2,100.00 in I4
it would display INELIGABLE in I5
