Incremental Weekly Date formula with End of month

I need to increase show the date range by 7 days showing the month end date.
The date need to look like this
11-Nov-12 18-Nov-12 25-Nov-12 30-Nov-12 2-Dec-12 9-Dec-12 16-Dec-12 23-Dec-12 30-Dec-12 31-Dec-12 6-Jan-13

The formula i created calculates month end date but the futuristic dates are based off the month end date instead of the date present prior to the week of month end.

Please make it in an excel workbook with an expected output and attach it for our easy understanding.

I tried as suggested by putting the month plus (add page) IN THE HEADER, and it worked first time, then afterwards I just kept getting Page 1 when I printed 2 pages. Have included the sheet I want printing with a different day on each page - for an appointment

diary for my daughter. Perhaps I pressed a wrong button somewhere, I often tweek to experiment.
