    Multiple IF Functions or VLOOKUP functions

    I have a data entry page for all the equipment in our building, problems with the equipment and the date. I am then trying to make a sheet that brings all of it together. It would display the date, then the equipment that is getting fixed and the issue it is having. I tried using IF statements and VLOOKUP but could only get 1st column data. Also I tried putting the date on top rather than on the far left column and not sure which is better. I uploaded the sheet for any help anyone could give. Thanks!
