Problem in creating List & sublist.

Hi I Want to create a list for eg. in the attachment in Service Description If I select Recyclable waste then it should give me 3 different Service period/unit & for Each unit there is fixed Service Ref# & price. which i am not able to link
1) create service list without duplicate.
2) link Service Period/Unit to Service Description
3) link Service Ref# & price to Service Period/Unit
All details are there in sheet 1 named "list".

Any help on this is really appreciated.
