Excel match and index function help???

dear all,


Please find my attachment here with.there you can find two sheets one is named data and the second one is result.
what i need is the information in the data sheet is to be transposed to result sheet in that same format.i tried match, index and vlookup formula's but i can 't get the result what i need.hence for your reference i did it manually.please anyone help me on this function or is there any simple way to solve this.
