Conditional format based on 4 criteria??

I'm thinking I want a conditional format but if your knowledge thinks I should achieve my results another way, please let me know

In cell L5, I have averaged H5:K5. What I would like to do is: if F5 = FT and the result in L5 is < 30 shade blue....if f5 = PT and the result in L5 is > 30 green. I have attached a sample.
