Vlookup & Hlookup Combination

I need help with a formula that can do Vlookup and Hlookup together. I am attaching the spreadsheet of what I am trying to do.

1. Look up the value in cell A2 in A5:L5.
2. Look up the value in cell B2 in the column previous to the value cell identified in 1.
3. Return the corresponding value in the next column to the column in 2.

Hence, for example given in the spreadsheet, for a note rate of 5.125 and prime rate of 4.75, the value should be 6.
