    Try this formula
    Formula: copy to clipboard 
    =IF(Z1405<=9,"Normal",IF(AND(Z1405>=10,Z1405<=13),"Mild",IF(AND(Z1405>=14,Z1405<=20),"Moderate",IF(AND(Z1405>=21,Z1405<=27),"Severe",IF(Z1405>=28,"Extremely Severe")))))
