Add information from 1 table automatically into another.

Hi all,

Earlier this week I also posted a question and was helped tremendously.
However, I tried adding more into the formula and was not able to get the right combination.

In the testfile I added, from A19 the previous part can be seen.

I wrote my wants in yellow and hope someone can help me to add to my file.
