I am attempting to Create a list based on the values in column A being true - Column O but only return said values

I don't understand what you want? R3 is empty and why would you use VSTACK with one array?

Doesn't really clarify why you are trying to use VSTACK with one array? It serves no purpose.
You don't want VSTACK
Formula:
=CHOOSECOLS(FILTER(A2:P13,O2:O13="TRUE"),1,16)

An alternative using HSTACK
Formula:=LET(f,FILTER(A2:P13,O2:O13="TRUE"),HSTACK(TAKE(f,,1),TAKE(f,,-1)))
