countifs gets wrong results

Hi Everyone -

Formula in Col B. Showing 2 for all. It should only show a 1 for Sam, 0 for the others in Col A, as Sam is showing an error in column E.

=COUNTIF($D$2:$D$6,A2)+COUNTIF($E$2:$E$6,"=Yes")

Tried Sumproduct, Vlookup, Sumifs.

I'm at a loss.
