If range contains text, based on a reference cell. Index/Match? (Microsoft Forms Data)

Hello Everyone! I have a Microsoft form set up where users can check multiple boxes. Does anyone know how I could view the results based on the user ID?

Each question has changed a little over time but I have kept the left 4 characters the same to use as a question ID. Using that ID, I'm hoping to enter a user ID and be able to see the questions that were checked off and the ones that were not.
